The Director of Population Health is responsible for strategic design, implementation and evaluation of population health initiatives at Cottage Health. The director will be responsible for understanding community needs, developing community-level interventions, and collaborating to implement initiatives for defined populations. The position reports to the Vice President of Marketing and Population Health.

 

MAJOR ACCOUNTABILITIES / SPECIFIC JOB COMPETENCIES


Population Health Strategy and Communications: Establish a comprehensive population health strategy and program in collaboration with VP that supports the organization's strategy, vision, values, and goals. Draw from expertise of effective population health/public health strategies/interventions to help guide Cottage Population Health programs and initiatives. Integrate population health principles and practices throughout Cottage Health. Support VP in communicating progress to internal and select external audiences. Attend department meetings and represent Population Health as needed in internal meetings and across departments. Participate in community-based coalitions, and develop partnerships to further population health goals.


Department Infrastructure & Project Management: Lead efforts to define and standardize department policies, procedures, and operating practices. Develop plans and timelines in collaboration with VP and communicate changes or delays.


Program Development, Implementation & Evaluation: Understand the pressing population health needs of the community and partner to create innovative solutions. Drawing on knowledge of evidence-based practices/solutions, design and oversee development, implementation and evaluation of population health initiatives and Cottage Health’s Community Benefit program. Facilitate evaluation planning and data collection systems to support initiatives. Utilize process evaluation and continuous improvement to adapt to outcomes/results in real-time.


Data and Resource Management: Partner with community organizations to establish a data-sharing platform. Develop and maintain online tools and resources, including a learning lab and Cottage Data2Go, to build partner organization capacity and evaluation models.


Research: Identify and lead research projects in partnership with the Cottage Health Research Institute. Adopt national best practices for reporting, disseminating, and publishing findings to internal and external audiences.

 

QUALIFICATIONS


Master of Public Health, Master of Public Administration, or Master of Health Administration. Basic computer skills including MS Word, Excel, PowerPoint, and Outlook. Familiarity with online secondary data sources such as County Health Rankings, Community Commons, etc. 5-7 years designing, implementing and evaluating population health programs that address risk factors, diseases, and conditions in health care and community settings
